Violations (Inspector Comments):
The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.
Bulk rice hopper doors are open and dirty mixing bowls and utensils are observed on the hopper lids exposing foods to contamination. (Corrected - dirty bowls and utensils were moved to ware washing area.)
Observed fountain soda syrup bag in box food stored directly on the floor in rear storage area, rather than 6 inches off of the floor as required. (Corrected.)
Cooked grilled chicken food was held at 96 degrees Fahrenheit, in the hot well located in the grill area, rather than 135 degrees or above, as required. (Corrected.)
Loose rubber door gaskets observed on the counter height freezer unit located to the left of the flat grill. Observed accumulation of frozen condensate on interior surfaces.
The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was less than 50 ppm, rather than 200 - 400 ppm, as stated on the manufacturer's use directions. Provided training. (Corrected.)
Fountain soda dispensing nozzles, a food contact surface, was observed to have food residue and mold-like substance, and was not clean to sight and touch. (Corrected.)
Observed stovetop with an accumulation of dirt, food residue, debris on non-food contact surfaces.
Observed walk-in cooler shelving with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces.
Automatic paper towel dispenser at the preparation hand-wash sink had towels but was not dispensing. (Corrected.)
Light shield over vulcan hot hold has a very heavy accumulation of static dust.
