The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.
An open employee's beverage container was observed in kitchen and sushi areas, which are food preparation areas.
Food employees observed in kitchen/food preparation area, not wearing proper hair restraints, such as nets, hats, or beard covers.
Food utensils in rice cooker area observed in a container of water, which is not maintained at 135 degrees Fahrenheit.
Observed in-use knives, cleavers, tongs, stored in direct contact with surfaces that are not clean and not easily cleanable and sanitized.
Observed wiping cloths in kitchen and sushi bar areas, not being stored in sanitizer solution.
Food facility Person in Charge not able to produce records showing that raw or partially cooked ready-to-eat fish has been frozen by approved supplier as required, before use or sale.
Dairy whipped topping food was held at 70 degrees in the kitchen area rather than 41 degrees or below, as required.
Refrigerated ready-to-eat time/temperature control for safety food (cut leafy greens and other vegetables) prepared in the food facility and held more than 24 hours, located in the glass door cold holding unit, is not being date marked.
Time in lieu of temperature being used in the food facility to control ready-to-eat potentially hazardous foods (cooked rice) without written procedures or documentation to verify disposition of food. (Corrected.)
Interior surface of chest/freezer/microwave is cracked and/or repaired with materials (tape) unapproved for food equipment.
An in-use cutting board has deep cuts and staining.
Dishes and utensils observed in three handwash sinks located in both kitchen and sushi areas, indicating uses other than handwashing.

The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.
Cigarettes, cigarette butts, and ashtray found on the floor in the back storage unit during the inspection.
Raw Food items (chicken, pork, beef, shrimp) in the walk-in cooler stored above vegetables.
Cardboard is being used to line tops of chest freezers, which is not an approved material, due to it being absorbent.
Raw chicken observed thawing at room temperature in the third bay in the three-compartment sink area, which is not an approved thawing method.
Old food residue, dishes, and utensils, observed in the handwash sink, indicating uses other than handwashing.
The handwash sink in the employee bathroom and in the back prep area does not have single-use towels, continuous towels, or air drying service.
No sign or poster posted at the handwash sink in the employee bathroom area to remind food employees to wash their hands.
Flooring in the back storage area with food debris, dirt, and grime, and in need of cleaning. Floor drains at three-compartment sink, prep sink, and grill area are all dirty with accumulation of grease, pink and white matter, and in need of cleaning.
