  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

  • Raw beef and raw pork stored above plantains, green bananas, carrots and onions in walk-in cooler, Corrected. Par cooked pork in the walk-in cooler observed stored under raw meats. Corrected.

  • Observed peeled plantains stored in non-food grade buckets (blue Lowe's bucket).

  • Pork belly food was held at 131 degrees Fahrenheit in the steamtable area, rather than 135 degrees, as required.

  • Observed a mixture of fresh garlic in oil sitting on the counter outside of refrigeration. Mixture was holding at 68 degrees Fahrenheit rather than 41 degrees or below. Corrected.

  • Refrigerated ready-to-eat foods in the walk-in cooler (lunchmeat, ground cooked meats) held more than 24 hours are not being date marked. Corrected.

  • Knives (stored on magnetic bar), a food contact surface, was observed to have food residue and was not clean to sight and touch. Corrected.

  • Handwashing sink in the kitchen area has a leak and is turned off when not in use.

  • Paper towel dispenser empty at the handwash sink in the warewash and kitchen area. Corrected.

  • Observed two ceiling tiles in the kitchen area with water stains, one is sagging, indication of a water leak at some point.

  • Personal medications such as Tylenol, Excedrin, and Vicks stored with food and on top of the microwave. Corrected.

Violations:

  • The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

  • Food employee observed donning single-use gloves without a prior hand wash.

  • Observed a container of beef jerky with white mold like matter out for sale to customers. Pulled product and placed behind counter.

  • Raw shelled eggs stored above ready-to-eat packaged foods in the grab-and-go case.

  • Apples observed in the grab-and-go case not wrapped or with a sign stating to wash before consumption.

  • Commercially processed hot dogs, tornados, cheese, bacon, refrigerated ready-to-eat, time/temperature control for safety food, located in the small counter fridge, and held more than 24 hours, is not marked with the date it was opened.

  • Single service forks and spoons on the coffee bar stored in a cup, not inverted.

  • Food facility has an employee who held a Certified Food Manager certificate, however, the certificate has expired and is no longer valid.

  • Observed outside area by trees and dumpster area with excessive debris, wood, and pallets. Area needs to be cleaned up to prevent pest harborage.

  • Both customer bathrooms and employee bathroom that are unisex without a covered trash receptacle.

  • Paper towel dispenser empty at the handwash sink, in the customer restroom, and in the front counter area. Corrected.

  • Ceiling tiles in the backroom are sagging and water damaged, and in need of repair.

  • Flooring in the walk-in cooler under the tea shelf with standing liquid and in need of cleaning.

  • Handwashing sinks/toilets/urinals are not being cleaned as often as necessary to keep them clean.

Rice and Beans Restaurant II, West Manchester Township

Violations:

  • The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 50-11 ppm, as required.

  • Hood and hood filters with accumulation of grease and in need of cleaning. Filters should be cleaned in between professional cleanings by restaurant.

  • Food facility is using Chlorine Sanitizer at an extremely high concentration of 200 ppm, not approved in the Code of Federal Regulations for food contact sanitizing at his level.

  • Facility does not have written procedures for employees to follow when responding to an event involving vomitus or fecal matter discharge onto surfaces within the facility. (Provided a copy via email.)

  • Facility had a change of owner seven months ago and the facility does not have a certified food manager.

  • The person in charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

  • Raw beef, chicken, fish, and pork in the walk-in raw meat cooler area and in the stand-up freezer in the kitchen area stored open with no covering.

  • Raw pork stored above raw fish in the walk-in cooler. Corrected.

  • Observed deeply-scored butcher block cutting boards not resurfaced or discarded as required.

  • Observed clean food equipment and/or utensils in clean dish area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting).

  • Hot water knob at the handwashing sink in the prep/warewash area is broken and in need of repair. Hot water is being turned off to sink.

  • Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration.

  • Bone saw, slicer, hand mixer, and top interior of microwave, a food contact surface, was observed to have food residue and was not clean to sight or touch.

  • Soap was not available at the handwash sink in the front counter area.

  • Paper towel dispenser empty at the handwash sink in the prep area.

  • The handwash sink in the front counter area does not have single-use towels, continuous towels, or air drying device.
