Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health March 7 through March 13:

Shiloh General Store, 28 E. Main St., Shiloh, March 7. Person in charge (PIC) did not ensure that employees observed foods as they were received (critical repeat, corrected during inspection). PIC verified that temperatures are not being taken on food products received via cold shipments. PIC was properly educated on proper procedures for checking foods received.

Family Dollar Store #23599, 1219 Ashland Road, Mansfield, March 8. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed front merchandise cooler reading 46 degrees.Adjust cooler to 41 degrees and monitor unit to meet required cold holding temperature. Perishable foods like milk shall be monitored and discarded if not at temperature more than four hours.

Dollar General Store #4638, 1280 Lexington Ave., Mansfield, March 8. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected during inspection). Observed candy stored on end cap display in aisle one directly below chemical/non-consumables (air fresheners, deodorant, foot cream and shampoo). PIC removed candy from display.

Food in an RFE must be embargoed due to adulteration, misbranding or baby food or formula is expired (critical, corrected during inspection). Observed can of Gerber Good Start formula on retail shelf with use by date of March 1. PIC opened and discarded contents.

Tamara K. King/Bostic Time, 4218 Park Avenue West, Mansfield, March 8. No air gap or approved backflow prevention device on the plumbing system (critical repeat). At re-inspection observed an unapproved backflow, ASSE 1024, installed. Facility states they have contacted a plumber to install a compliant backflow. RPH plumbing department indicated no plumbing permit has been obtained as of March 7. Correct by March 22.

Family Dollar Store #21617, 941 Springmill St., Mansfield, March 11. Person in charge did not ensure that employees observed foods as they were received (critical, corrected during inspection). PIC stated that she was unaware that dented cans (compromised) must be pulled from sale. Ensure that call canned items with dents that lead to sharp edges, dents on seams (side and top/bottom) are pulled from sale. Food package(s) received in poor condition (critical repeat, corrected). Observed cans of cream of mushroom soup, diced tomatoes, pork and beans and mandarin oranges to be dented and the quality of the contents cannot be guaranteed. PIC voluntarily discarded the cans.

TownePlace Suites, 1125 Guest Court, Ontario, March 12. Manual wash solution not being maintained at 110 degrees or above (critical repeat, corrected during inspection). Observed employee washing equipment and manual wash water to be 105 degrees. Employee added more hot water and increased temperature to 110.

Family Dollar Store #23628, 1314 Park Avenue West, Mansfield, March 12. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC could not state temperature parameters for holding or receipt of TCS foods. Ensure TCS products are maintained at 41 degrees or below at all times. Education was provided. Food package(s) received in poor condition (critical, corrected during inspection).

Observed one can of garbanzo beans to be dented. PIC voluntarily pulled item from sale. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected during inspection). Observed Pedialyte and protein shakes on the clearance rack under various chemicals and medications. Ensure all food products are stored above or completely separated from chemicals at all times.

Meijer Express #126/Gas Station/C-Store, 1321 N. Lexington Springmill Road, Ontario, March 12. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed American cheese with March 11 discard date. Employee verified date is incorrect as sticker did not get removed from container and date marked for discard March 13. Food contact surfaces not easily cleanable (critical, corrected). Observed small pair of scissors used to cut plastic on frozen pizzas not easily cleanable as scissors cannot be fully separated for effective cleaning. Employee removed scissors from use.

Richland Skyline Inc., 1320 N Lexington Springmill Road, Mansfield, March 12. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed TCS foods (diced tomatoes, grilled chicken, feta cheese crumbles, cream cheese) in top portion of prep cooler to have internal temperatures above 41 degrees. PIC placed tomatoes, grilled chicken and feta cheese in walk-in cooler to rapidly cool. PIC discarded cream cheese.

Subway #30929, 1521 Lexington Ave., Mansfield, March 13. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Observed both current employees to not have employee illness agreements signed on location. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed deli slicer to have visible residue on the food contact portions. Insufficient air gap between the flood rim and the water supply inlet (critical). Observed no air gap under the soda machine. Correct by re-inspection March 20.

Madison South Elementary, 700 S. Illinois Ave., Mansfield, March 13. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Pizza observed on food cart uncovered and subject to sneeze, splash, dirt and dust. TCS foods not being cold held at the proper temperature (critical, corrected).

Tossed salads held cold on food line measured at 44 degrees with next serve time in 20 minutes. Listed item was relocated to walk-in cooler. TCS foods not being cold held at the proper temperature (critical, corrected). Cold-held tossed salad inside two door True cooler measured 47 degrees. Listed TCS items were relocated to walk-in cooler.
