Richland County restaurant inspections: critical violations March 1-7

The following restaurant inspections with critical violations were conducted by Richland Public Health between March 1 and March 7:

Wokano, 2284 W. Fourth St., Ontario, March 1. Raw shell eggs not maintained at 45 degrees or below (critical, corrected during inspection). Observed eggs stored in deep metal pan on hibachi carts to have internal temperature of 47 degrees. Person in charge (PIC) had employees place eggs back into cooler to cool to 45 degrees or below. Discussed ensuring TCS (time/temperature controlled for safety) foods are not stocked on hibachi carts until hibachi orders have been received.

Tamara K. King/Bostic Time, 4218 Park Avenue West, Mansfield, March 1. No air gap or approved backflow prevention device on the plumbing system (critical repeat). At time of inspection, backflow observed on plumbing system for facility not correct for system. At re-inspection, discussed contacting chief plumbing inspector at Richland Public Health regarding corrections notice. Correct by March 9.

Discount Drug Mart #59, 25 Briggs Drive, Mansfield, March 4. Improper display of poisonous or toxic materials offered for retail sale (critical, corrected during inspection). Observed hand sanitizer and hand lotion stored directly above gum on display near cash register at front of store. PIC moved hand sanitizers and lotion to different location.

Family Dollar Store #23599, 1219 Ashland Road, Mansfield, March 6. TCS foods not being cold held at the proper temperature (critical). Observed front merchandise cooler reading 46 degrees. Adjust cooler to 41 degrees and monitor unit to meet required cold holding temperature. Perishable foods like milk shall be monitored and discarded if not at temperature. Correct by March 8.

Gigi's Tacos and Margaritas, 1111 W. Fourth St., Mansfield, March 7. Handwashing sink not accessible (critical, corrected during inspection). Items stacked in front of hand sink in bar. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Thawing raw eats stored above ready-to-eat foods in two door cooler. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Mushrooms in two-door cooler and chips in dry storage were uncovered and not in use. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Can opener blade observed with dried on residue. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Warewashing machine was at zero ppm. PIC discontinued use and is using three-compartment sink. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Spray bottle under warewashing machine with cleanser in it was not labeled with common name.

Everbowl, 2074 Walker Lake Road, Mansfield, March 7. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed fruit slicer on prep surface and additional slicers in clean storage to contain visible dried residues. PIC states new slicers have been ordered; removed observed slicers from use. PIC instructed employee to use knives until new slicers arrive. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected during inspection). Observed everoats in tall reach-in cooler not date marked. Employee verified oats were made previous day and date marked at time of inspection.

Cinemark 14, 2355 Walker Lake Road, Ontario, March 7. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed opened bags of hot dogs date marked for discard on March 3 and March 6. PIC discarded during inspection.

Dollar General Store #4673, 1660 W. Fourth St., Mansfield, March 7. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed cartons of raw eggs on a shelf above bottled drinks in the back room cooler. PIC moved eggs to the bottom shelf inside the cooler. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed lighters above candy and gum at the front registers. PIC removed lighters from displays. Food in an RFE must be embargoed due to adulteration, misbranding, or baby food or formula is expired (critical, corrected during inspection). Observed a can of Enfamil baby formula with a use by date of November 2023. PIC emptied contents into the trash.

J & J Food Mart, 305 Marion Ave., Mansfield, March 7. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). PIC states they do not use a sanitizer solution when washing dishes. Reviewed proper warewashing procedures. PIC pulled bleach from sales floor to use to sanitize utensils. Reviewed bleach cannot be scented.
