Richland County restaurant inspections: critical violations

The following restaurant inspections with critical violations were conducted by Richland Public Health between March 27 and April 2:

Subway #30929, 1521 Lexington Ave., Mansfield, March 27. Insufficient air gap between the flood rim and the water supply inlet (critical repeat). No change observed during re-inspection. Richland Public Health Plumbing Department has been made aware of the situation and will be on site. Correct by April 2.

Garden Gateway Café By Buehler's, 50 Trimble Road, Mansfield, March 28. Foods from unapproved sources (critical). Observed frozen custard from another facility being sold in the establishment. Wholesale registration from Ohio Department of Agriculture unable to be verified at the time of inspection. Discussed food from an unapproved source cannot be sold at a licensed food service operation. Correct by April 4. Presence of live insects (critical). Observed ants under the three-compartment sink. Person in charge (PIC) stated licensed pest control company will be contacted. Correct by April 4.

Penn Station, 2166 Walker Lake Road, Mansfield, March 28. PIC unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). Multiple critical violations observed. All critical violations corrected and education was provided. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed bag of sugar in back storage area store on storage shelf opened and uncovered/unprotected from potential contamination. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed visible residue on cutting surface of manual can opener. Improper storage of poisonous or toxic materials (critical). Observed working bucket of sanitizer stored on prep surface beside cutting board and bottle of window cleaner stored hanging on shelf above boxes of potatoes. PIC relocated both the bucket and bottle to a separate area. Insufficient air gap between the flood rim and the water supply inlet (critical, corrected). No air gap observed for drain lines from fountain beverage machine and ice maker at floor drain located inside cabinet below machine. An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch.

Dollar General Store #14526, 40 Cleveland St., Butler, March 28. Person in charge not ensuring that employees verify foods are approved and received properly when delivered during non-operating hours (critical, corrected during inspection). PIC reported that staff do not check temperatures of refrigerated items upon receipt of delivery. Proper instruction was given on taking temperatures. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected). A copy of the agreement was provided.

Crossroads Church, 1188 Park Avenue West, Mansfield, March 28. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat). Observed whipped cream in the cooler to be opened without date mark. PIC verified when the whipped cream was opened and date marked at item of inspection.

Pumpkin Seed Bulk Food Co., 1625 Executive Drive, Mansfield, April 1. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed bottles of dish soap and fabric softener on a shelf above a bag of flour.

Bob Evans Restaurants LLC #485, 2100 W. Fourth St., Mansfield, April 1. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Hand sink by register observed to have coffee and grounds dumped in it. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Corn in walk-in cooler observed uncovered. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Uncovered and unattended biscuits observed by registers. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Utensils in clean area observed to have dried on food residue. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Can opener blade observed with dried on residue. TCS foods not being cold held at the proper temperature (critical, corrected). Items in preparation area had been out for an undetermined amount of time; had internal temperatures of 57-60 degrees. PIC voluntarily discarded 2 pounds each of beef and chicken gravy, 3 pounds of mashed potatoes, 5 pounds of cooked ham, 10 pounds of cooked turkey, 3 pounds of potato soup and 5 pounds of cooked carrots. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Spray bottle observed with unknown chemical and was removed. Food contact surfaces not easily cleanable (critical, corrected). Container of cut salad observed in tub split down into the food contact area. PIC moved salad to another container.

Charley’s Philly Steaks, 2120 W. Fourth St., Ontario, April 2. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed milk held in insulated pitcher to have an internal temperature of 46 degrees. PIC discarded milk. Food thermometers are not accurate (critical, corrected). Observed digital probe food thermometer not accurate; thermometer read 46 degrees when actual temperature of the food was 51 degrees. PIC removed thermometer from use for calibration.

7-Eleven Store No. 36173 A, 26 W. Main St., Lexington, April 2. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed a package of raw bacon on a shelf above ready-to-eat food in the retail display cooler. Improper storage of poisonous or toxic materials (critical, corrected). Observed a display of lighters on the front counter above packaged food.

Dollar Tree Store #4881, 1448 Lexington Ave., Mansfield, April 2. Food package(s) received in poor condition (critical repeat, corrected during inspection). Observed one can of chicken gravy and one can of tomato soup to have dents on the upper seals. PIC removed from sale. Presence of live rodents (critical). Observed rodent droppings behind palettes labeled candy-food. Facility had service report on hand for last pest control service. Facility is to thoroughly clean and sanitize the area and contact a licensed pest control operator. Must have report or invoice available for review during re-inspection on April 9.

Main Street Pit Stop, 56 W. Main St., Lexington, April 2. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed raw bacon on a shelf above ready-to-eat products inside the walk-in cooler.
