The following restaurant inspections with critical violations were conducted by Richland Public Health between Jan. 30 and Feb. 1:

AVI Food Systems Inc./American Standard, 41 Cairns Road, Mansfield, Jan. 30. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Sausage rolls date marked Jan. 24 were observed stored in cold holding food vending machine. Listed item was voluntarily discarded. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Seven sandwiches date marked Jan. 29 were observed in cold-holding food vending machine. Listed food items were voluntarily discarded.

AVI Food Systems Inc./Bunting Bearing Corporation, 153 E. Fifth St., Mansfield, Jan. 30. TCS foods not being cold held at the proper temperature (critical). Food inside vending machine measured 44 degrees. Vending machine must hold temperatures at or below 41 degrees. Correct by Feb. 13.

Hong Kong Buffet, 331 N. Lexington Springmill Road 125, Mansfield, Jan. 31. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Walk-in cooler had cake, potatoes and chicken uncovered. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Raw shell eggs in walk-in cooler observed above ready-to-eat foods. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Bus tubs used to store clean utensils in ware washing area had food residue and standing water. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Clean cutting boards observed stored on floor in back prep area. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected). Sanitizer buckets at greater than 200 ppm. Improper cooling of TCS food (critical, corrected). Rice put in walk-in at 9:30 a.m. was 75 degrees at 11:50 a.m. Person in charge (PIC) voluntarily discarded less than one half a pan of white rice. TCS foods not being hot held at the proper temperature (critical repeat, corrected). Buffet line foods (salt & pepper shrimp, coconut chicken, sweet and sour chicken, and dumplings) being held under 135 degrees. PIC had rapidly reheated to above 165 degrees. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat, corrected). TCS Items in walk-in cooler that were prepared over 24 hours ago were not date marked. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Sanitizer buckets not labeled with common name.

VFW 9943 - Mohican, 806 Grace St., Mansfield, Jan. 31. Presence of live insects, rodents, and other pests (critical). Observed mouse droppings on back shelf under microwave and around floor, under equipment. Correct by Feb. 9.

Ontario AMVETS Post 31, 2943 Park Ave. West, Mansfield, Feb. 1. Handwashing sink not accessible (critical, corrected during inspection). Observed handwashing sink filled with cleaning supplies and in accessible to employees. CS foods not being cold held at the proper temperature (critical, corrected). Observed opened container of cranberry juice with an internal temperature of 50 degrees. PIC voluntarily discarded cranberry juice. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed cut lettuce and opened mozzarella cheese held in refrigerator in kitchen area not date marked. PIC voluntarily discarded items.

Nelly's Hideaway LLC, 3717 Park Ave. West, Ontario, Feb. 1. Person in charge (PIC) did not ensure employees were effectively cleaning their hands as required (critical, corrected during inspection). PIC stated that hands are sometimes washed at the three-compartment bar sink. Person in charge did not ensure that employees observed foods as they were received (critical, corrected). PIC stated temperatures of received product are not being taken to ensure deliveries come in at temperature. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected). PIC unable to demonstrate knowledge of cleaning and sanitizing (critical repeat, corrected). PIC stated that ice scoops are being cleaned once every few days. Handwashing sink not accessible (critical repeat, corrected). Observed handwashing sink to be blocked by rolling cart. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed cut potatoes in the small make line to be uncovered. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected). Observed the potato slicer and a knife stored in the clean knife holder to both have visible residue. Improper storage of poisonous or toxic materials (critical repeat, corrected). Observed chemical cleaners to be stored hanging on the mixer/liquor rack where cross contamination may occur.

Oak Grove Manor, 1670 Crider Road, Mansfield, Feb. 1. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Observed a bowl of washed grapes in the walk-in cooler that was not covered. PIC voluntarily discarded grapes.

Empress Express, 1041 Park Ave. West, Mansfield, Feb. 1. PIC unable to demonstrate knowledge by having no critical violations (critical repeat, corrected during inspection). Person in charge did not ensure employees are properly cooking TCS foods and/or using thermometers as required (critical repeat, corrected). PIC stated he does not take temperatures while cooking and reheating. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Observed peeled/prepped onion in the walk-in cooler without a cover and a pan of lard in the upright cooler without a lid. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed deli slicer and knife stored in a clean area to have visible residue. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed wontons in the makeline to have an internal temperature of 51 degrees. TCS foods not being cold held at the proper temperature (critical repeat, corrected). Observed container of garlic in oil with an internal temperature of 65 degrees. Improper storage of poisonous or toxic materials (critical, corrected). Observed beta fish food, oven cleaner and soap being stored in junction with potatoes and vegetable oil. Food contact surfaces not easily cleanable (critical, corrected). Observed bean sprouts being stored in a takeout plastic bag. PIC was unable to verify if that bag is food grade. Food contact surfaces not easily cleanable (critical, corrected). Observed paper towels being used as food contact surfaces for precooked egg rolls, rendering the surfaces no longer smooth, durable, non-absorbent or easily cleanable.

V & M Restaurant, 104 Main St., Bellville, Feb. 1. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection).
