No assigned person in charge to monitor and maintain the facility. Person in charge arrived after inspection and was unable to demonstrate food safety knowledge or principles. Facility was using two compartment sink to wash dishes. Product in coolers not properly holding temperature. Ant and roach spray in kitchen. Spray bottle with yellow liquid with no identifying label. Facility had no thermometers. Gaps around door leading to the outside of the kitchen area. Numerous small flying insects throughout the kitchen. Food items like waffle mix stored directly on the ground. Wiping clothes stored on the counters in the kitchen area. Single-use items like napkins and plates stored on the ground. Cabinets throughout kitchen with heavy debris buildup. Numerous small holes throughout kitchen walls. Employee personal items stored on shelves next to and above food items. Lack of refrigeration in kitchen area.
