Door to the facility not fully sealed. Roof had multiple areas where mesh had been placed over penetrations and holes. Significant rodent droppings throughout facility behind food service equipment. Five dead rodents inside glue traps inside the kitchen and one dead rodent under prep table. Instructed person in charge that traps must be checked regularly and pests removed. Compartment sinks with plumbing lines leaking. Cracks in crevices at base of stairs. Significant gap in the ceiling.

Result: Immediate closure

Facility used steel wool to seal a hole. Door is not fully sealed. Mesh used to seal holes in roof. Significant rodent droppings throughout the facility behind food service equipment and inside coolers and freezers. Rodent droppings along the perimeter of the kitchen area such as under dish storage and hand-washing sink. Observed five dead rodents inside glue traps. Significant gap in the ceiling.

Result: Immediate closure
