Inspector Notes: Observed TCS food cold (bacon) holding above 41F in the reach in cooler near the back of the kitchen.

Inspector Notes: Observed hot water in bathroom to be turned off and only cold water available. PIC stated that water was turned off because of someone else using their hot water. Observed green bucket to be stored in handwashing sink in front food service. Observed containers of oil and soy sauce to be stored on the floor in the front area of the facility. Observed old food debris within cold holding units(towards the back) throughout the facility. Observed grease and debris along the walls behind the stove and wok station. Observed discolored and damaged ceiling tiles in the main kitchen and dry storage area. Observed walls under sink in bathroom to be damaged. RCA: Facility shall thoroughly cleaned and walls and cleaning fixed.

Inspector Notes: Rule: Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. Inspector's Note: Observed multiple bowls and cups of employee food stored in reach in cooler above and along side food for sale. Notified PIC. PIC discarded all items. Observed multiple food items in facility that had no date, and were not prepared day of. Observed multiple food items with date labels that were outside of the 7 day safe holding time (Pork - 12/12. Soup - 12/29, discard date over 7 days). Notified PIC. PIC discarded all items that were not dated our past discard date. Observed multiple items stored in reach in cooler and walk in cooler that had no label on them, and could not be readily identified. Notified PIC. PIC discarded all items. Observed that score from two inspections previous was being displayed in restaurant, and reflected a score higher than previously assigned. Observed walk-in cooler with a damaged door that is not self-closing and water damage to the walls and ceilings with erosion at the joining points as well as cracks in walls.
