Public Records: Guernsey County restaurant inspections

The following are restaurant inspections with critical violations, as conducted by the Guernsey County Health Department, from Nov. 21 to 30.

Nov. 21

Buffalo Duchess #1210, 11830 Clay Pike Road, Buffalo. Several items in hot holding units including chicken bites, fried bologna sandwiches, cheeseburgers, and Italian sub sandwiches with internal temperatures of 109 to 119 F. Time/temperature controlled for safety (TCS) foods must be hot held at 135 F or above. Pre-packaged Italian pasta salad and cheese trays were not labeled with ingredients or allergen declarations.

Dough Boyz Pizzeria LLC, 56500 Vocational Road, Buffalo. Gallon jug of whole milk in cooler with use by date of Nov. 17; corrected during inspection. The water hose connected to the mop sink faucet was resting directly in the sink with no air gap or backflow prevention device provided.

Southeastern Ohio Regional Medical Center, 1341 Clark St., Cambridge. Cheddar cheese in a bowl was sitting out at room temperature on the breakfast serving line and registered 64 F at the time of inspection; corrected during inspection. TCS food must be held at 41 F or below during cold holding.

Dennys Restaurant, 2031 Southgate Parkway, Cambridge. Employees are not informed in a verifiable manner of their responsibility to report information about their health; corrected during inspection. Employees were switching gloves and not washing hands. An employee in the dishwashing area was not washing their hands after being contaminated. Handwashing sinks are being used for other purposes and drinks are being dumped in them. A whisk and spoon were sitting in the sink. Scrambled eggs in the prep unit registered 44.9 F; corrected during inspection. TCS food must be held at 41 F or below during cold holding. Shell eggs in the prep unit registered 54.6 F; corrected during inspection. Eggs must be kept at 45 F or below.

Wendy's of Cambridge, 1708 Southgate Parkway, Cambridge. Employees are not informed in a verifiable manner of their responsibility to report information about their health. The manager could not locate the employee health policy at the time of inspection; corrected during the inspection. An employee did not wash hands after mopping and before putting on gloves; corrected during inspection. Sanitizer concentration in the 3-compartment sink registered 100 parts per million (ppm) at the time of inspection; corrected during inspection. Quat concentration must be at least 200 ppm.

McDonalds, 2022 Southgate Parkway, Cambridge. An employee started their shift and did not wash hands before going into the food area; corrected during inspection.

Subway North, 1335 Clark St., Cambridge. Employees are not informed in a verifiable manner of their responsibility to report information about their health; corrected during inspection. Employees were coming into the building after a break and did not wash their hands before starting work; corrected during inspection. The handwashing sink was dirty and is being used to dump squirt bottles. Meatballs in the hot holding unit registered 122 F; corrected during inspection. TCS foods must be hot held at 135 F or above.

Epic Center, 10015 Brick Church Road, Cambridge. Raw shell eggs stored above pre-cooked scrambled eggs in the walk-in cooler; corrected during inspection.

Nov. 22

Creno's Pizza, 224 Dewey Ave., Cambridge. A spray bottle of sanitizing solution was stored on a shelf with food products. Ortho Home Defense insect killer was observed in the rear storage area.

Nov. 27

Cardinal Place, 163 Meadowpark Drive, Cambridge. A container of butter was stored at room temperature with an internal temperature of 67 F; corrected during inspection. TCS foods must be cold-held at 41 degrees or below. A spray bottle of peroxide-based cleaner observed on a shelf in the kitchen was not labeled with the common name of contents; corrected during inspection.

Cambridge Health and Rehabilitation Center, 1470 Wills Creek Valley Drive, Cambridge. Temperature logs for hot and cold holding were not accurate, logs show the same temperatures for different foods and days; corrected during inspection. Sanitizer strength in sanitizing bucket registered 0 ppm and water in the 3-compartment sink registered 0 ppm. The sanitizer container was low and there was air in the line. The person in charge could not find another Quat sanitizer container to refill the sanitizer. Until the container is replaced, PIC will have employees manually add sanitizer and use test strips to ensure proper sanitizing strength of 220 ppm is achieved.

Nov. 29

Guernsey County Senior Center - Senior Nutrition, 1022 Carlisle Ave., Cambridge. Noodles in the refrigeration unit were not date marked; corrected during inspection.

Bob Evans Restaurants, LLC #217, 2358 Southgate Parkway, Cambridge. The sanitizing concentration in the sanitizing bucket registered 0 ppm. The water was dumped and replaced but was still registering below 200 ppm. It was discovered that the lid on the sanitizer was not on. Once it was tightened, sanitizer strength registered at 200 ppm; corrected during inspection. Food contact surfaces shall be effectively sanitized and quat must reach a strength of at least 200 ppm.

Theo Brothers Inc. DBA Forum, 2205 Southgate Parkway, Cambridge. Items in the handwashing sink. The hot water temperature on the dish machine only reached 175 F. Hot water temperature must reach at least 180 F.

Ruby Tuesday #4358, 2250 Southgate Parkway, Cambridge. Ice in the handwashing sink in the grill area; corrected during inspection.

Nov. 30

The Airport Tavern LLC, 61561 Wintergreen Road, Lore City. Raw shell eggs stored above ready-to-eat items; corrected during inspection. TCS foods held for longer than 24 hours were not date marked including cooked chicken and deli meats. A spray bottle of blue liquid in the bar area was not labeled with the common name of contents.

Beech Grove Family Center, 60901 Beech Grove Lane, Cambridge. Cooked hot dogs, shredded chicken, cheese sauce, ground beef, and desserts were not date-marked. Elmer's Wood Glue and bottles of hydrogen peroxide were in drawers with utensils and equipment at the time of inspection.

Due to the large amount of restaurants with critical violations, the November reports will be broken into three separate articles.
