Public Records: Guernsey County restaurant inspections

The following are restaurant inspections with critical violations, as conducted by the Guernsey County Health Department from Dec. 1 to 9.

Dec. 1

360 Burger, 11201 Cadiz Road, Cambridge. Raw eggs above ready-to-eat food; corrected during inspection.

Dec. 4

Lisa's The Brunch Box LLC, 3394 Glenn Highway, Cambridge. Raw shell eggs and raw scrambled egg mix stored above ready-to-eat items. Quaternary ammonium solution in use in a 3-compartment sink had a concentration of 100 parts per million (ppm). Quaternary ammonium solutions for the sanitizing of food contact surfaces of utensils and equipment must have a minimum concentration of 200 ppm. Raw shell eggs stored on the counter, not under refrigeration. The internal temperature of the eggs registered at 64 F. Raw shell eggs must be maintained at 45 degrees or below. Time/temperature controlled foods for safety that have been prepared and frozen for later re-heating are not properly date marked when removed from the frozen state and placed under refrigeration. Spray bottles of chemicals in the service area of the kitchen are not labeled with the common name of contents or are incorrectly labeled.

Dec. 5

Downtown Arena, 1005 Wheeling Ave., Cambridge. Handwashing sinks were dirty and being used to dump beverages. The drain line from upstairs splashing wastewater onto Pepsi syrup boxes in the basement. The inside of the ice machine is dirty. Pumpkin pie was dated Nov. 21 and not discarded after seven days; corrected during inspection. Ant and roach killers were observed in the facility.

Stillion Center Market LLC, 11790 Cadiz Road, Cambridge. Raw shell eggs stored directly above ready-to-items in the retail display cooler and the refrigerator in the kitchen area. Coffee pot being washed in mop sink; corrected during inspection. Lighter fluid and propane cylinders stored above food items in the retail area; corrected during inspection.

Dec. 6

Buckeye Trail High School, 65555 Wintergreen Road, Lore City. The facility is using logs for hot holding, cold holding sanitizing strengths and dishwasher temperatures, but some logs do not appear accurate: cold holding temperatures are consistently the same for different foods over several days, cold holding temperatures in some cases were lower after being taken out of refrigeration unit, hot holding temperatures were the same for several different foods over several days, etc. The person in charge was not reviewing the sanitizing temperatures that were being documented and was not aware that the dishwasher was not reading at least 180 F for proper sanitizing. Temperature cooking logs showed that TCS food was cooked to 110 F. Rinse gauge on the dishwasher registered 141 F and the final rinse temperature registered 118 F. Hot water must reach at least 180 F to effectively sanitize dishes and kill harmful organisms that cause foodborne illness. According to dishwashing logs, wash, rinse and final rinse temperatures have been inconsistent and have not reached proper temperatures several times since August. Turkey and ham deli meat was not date marked.

Buckeye Trail Middle School, 65553 Wintergreen Road, Lore City. Employees are not informed in a verifiable manner of their responsibility to report information about their health; corrected during inspection. The pizza sauce was dated Sept. 29. According to PIC, the lid was run through the dish machine and the September date was an old date; corrected during inspection. Buffalo chicken dip was not date marked; corrected during inspection. The sneeze guard was missing on a section of the serving line.

821 Carryout LLC, 58901 Marietta Road, Byesville. A spray bottle of blue liquid cleaning solution near the 3-compartment sink was not labeled with the common name of contents.

Dec. 7

Carridge Market LLC, 3700 Clay Pike Road, Cumberland. The handwashing sink in the deli area used as a dump sink and is soiled with food and beverage residues. Raw shell eggs are currently being obtained from a small egg producer that is not registered with the Ohio Department of Agriculture. Raw bacon was stored above ready-to-eat trail bologna. The facility is reheating bulk, commercially processed soups for hot holding in crock pots. Deli meats in the deli display case and walk-in cooler are not being date marked with a seven-day time limit when the original packaging is opened. Super Tech lubricant spray and Minwax polyurethane spray are stored above clean utensils and equipment in the rear storage room.
