Desi Chulha scored a 32 in its Aug. 18 inspection with inspectors noting, in addition to the refrigeration problems and poor safety procedures, that the store had plumbing problems, grease buildup on the stove and employees without food handler certifications.

The 32 at Desi Chulha was the highest score in this round of inspections.

Sushi Sakana in Southlake on Aug. 30 received the second highest score with a 27. Inspectors noted unclean kitchen surfaces, a dirty walk-in cooler, a dirty floor at the sushi line and improperly stored food, among other violations.

Of note, Old West Cafe in Bedford was closed for a week due to plumbing problems that have since been fixed, and Pizza Hut at C24 inside DFW International Airport had a repeat violation for a mold-like substance.
