Marion County restaurant inspections: Aug. 1-16 critical violations

The Louisiana Department of Health performs regular inspections of restaurants and stores that sell food. These are the local results of those inspections.

The following restaurant inspections with critical violations were conducted by Marion Public Health between Aug. 1 and Aug. 16:

OK Café, 734 E. Center St., Marion, Aug. 1. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed clean plate to be dirty.

Paradice Cream, 189 Marion St., Waldo, Aug. 1. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed sauce in prep cooler not properly date marked.

Hong Kong Buffet 88 Inc., 1561 Marion-Mt. Gilead Road, Marion, Aug. 2. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Did not observe lids on food items in make table cooler.

Steak 'N Shake #430, 2081 Marion-Mt. Gilead Road, Marion, Aug. 2. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed soups cooling to have no protective lids or hoods over them.

Rally's #4054, 412 N. Main St., Marion, Aug. 2. Food not properly protected from contamination by separation, packaging, and segregation (critical repeat). Observed fried food in hot holding unit to not have lids to cover the fries and chicken. Foods need to be protected from contamination through the use of lids especially in their ready-to-eat stage.

Waldo Swim & Recreational Center, 275 W. Main St., Waldo, Aug. 11. Food employee(s) did not wash hands when required (critical, corrected during inspection). Observed employee donning single-use gloves without washing hands.

Kingston Residence of Marion LLC, 464 Jamesway, Marion, Aug. 11. Food package(s) received in poor condition (critical, corrected during inspection). Observed dented can in storage area. Person in charge (PIC) removed it from shelves and marked it to be returned.

Van Java Hibachi, 526 W. Center St., Marion, Aug. 12. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Observed raw meat to be stored above TCS foods in the stand-up cooler. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed food in stand-up cooler to not have lids on containers. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed sliced vegetables in stand-up cooler to not have a date mark on them. Presence of live insects, rodents and other pests (critical). Observed mouse droppings behind the bar along with at-home pest control devices. PIC needs to hire a licensed pest control operator. Correct by Aug. 19.

McDonalds Corporation #2217, 1600 Marion-Mt. Gilead Road, Marion, Aug. 16. Equipment food-contact surfaces or utensils are unclean (critical repeat, corrected during inspection). Observed dish that had been clean to have residue on the food contact portion of a container. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed carton of egg mix being held past its disposition date of Aug.15. PIC threw container away. Food contact surfaces not easily cleanable (critical, corrected during inspection). Observed container to have chips and cracks on it. PIC removed container and threw it away.

The Wyndmoor of Marion (OH) LLC, 308 Barks Road E., Marion, Aug. 16. Food package(s) received in poor condition (critical, corrected during inspection). Observed three dented cans in the storage room. PIC removed them to be sent back. Ready-to-eat, TCS food not properly discarded when required (critical, corrected during inspection). Observed applesauce being held past its disposition date. PIC threw it out.

ProMedica Skilled Nursing and Rehabilitation (Marion), 400 Barks Road W., Marion, Aug. 16. No person in charge present in food facility during inspection (critical). There is no designated person in charge for the kitchen area due to the manager being gone. At least one person per shift needs to obtain a person in charge certification in food protection. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat). Did not observe signed sick policies in the facility. Food package(s) received in poor condition (critical, corrected during inspection). Observed dented can in storage area. Employee removed it to be sent back. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Observed sanitizer bucket for sanitizing surfaces such as prep areas to not reach the correct ppm Switched to EPA-approved lactic acid food contact surface sanitizer. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). Employee stated that coffee pots are cleaned just once per week. Coffee pots should be cleaned at least once a day. Working containers of poisonous or toxic materials not properly labeled (critical, corrected during inspection). Observed chemical bottle with green liquid in it to not have the common name on the container.
