Between organic cotton, “period panties,” and lacy SPANX, the options are seemingly endless when it comes to underwear types. And it's hard to resist a flattering cut and fun color. But as much as you may want to prioritize style, it's important to think about comfort and vaginal health when it comes to your panty purchases. (P.S. - You can totally find cute and breathable underwear that are vagina-friendly—we've got ob-gyn-approved recommendations below.) But first, a little more on how exactly your underwear affects your vagina.

How do underwear materials affect vaginal health?

Being mindful of what your skivvies are made of can save you from a host of vaginal health issues, like infections and irritation, down the line. That's because yeast and bacteria tend to thrive in moist, dark places, says Alyssa Dweck, MD, an ob-gyn in Westchester County, New York, and certain underwear materials can create those environments. "Breathability is really important," she says. “Synthetic materials like Lycra and nylon can be confining and non-breathable and can lead to irritation, rashes and infection, especially if you’re already prone to those things.” That's not to say that if you wear underwear made of these materials, that it will cause an infection itself or that you're bound to get one. But there are materials that are airier and will make your vagina much, much happier and cut down on your risk.

So what material should my underwear be made of?

Cotton is the best choice when it comes to everyday wear, according to Dweck. It’s the most absorbent material and the best for promoting breathability.

Are organic underwear better for my vagina?

If you have super-sensitive skin, or you’ve been dealing with recurrent or persistent infections or irritation, opting for organic cotton or other natural materials may be helpful, says Dweck. Otherwise, conventional cotton is just fine.

Wait...is it better for my vagina to just go commando?

Occasionally forgoing undies altogether is even better than opting for cotton, says Dweck. If you’re prone to vaginal infections, sleeping without underwear may help to curb them. Nude sleepers rejoice!

What kind of underwear are best for working out?

It's fine, and probably even beneficial, to forgo cotton when you're exercising. Moisture-wicking materials will feel most comfortable, says Dweck, but you should still be sure to change out of your sweaty clothes as soon as possible post-workout. Going commando can be easier in this regard as well, as most workout bottoms either have a built-in liner or are designed to be worn without underwear anyway.

Are thongs a good choice?

The good news: thongs aren’t any worse than full-coverage underwear as far as promoting or decreasing the chance of infection or irritation—as with other styles, it’s ultimately the materials that matter. Still, if you opt for a thong, go for one that fits properly and has a cotton crotch panel, as a too-tight thong can lead to chafing, particularly if it’s a g-string style, says Dweck. (Ouch.)

Do pantiliners impact my vaginal health?

Pantiliners can be great for keeping discharge and light menstrual bleeding at bay, but Dweck advises against using them all the time, as it can also minimize breathability down there. Period underwear can be better option if you need some absorbency and stain protection (plus, they'll help you cut down on your use of disposable products).

15 breathable underwear options

Even when you have certain materials and a particular style in mind, the abundance of underwear options out there can still be overwhelming. A good rule of thumb is that if you see a material blend, make sure the most abundant material's percentage is as close to 100 percent as possible, says Dweck.

“Be mindful of the elastic bands at the waist and groin when choosing underwear, too,” Dweck says. “If they’re too tight, this can lead to further irritation, along with chafing.”

Try one of the following 15 options, approved by Dweck, to stay comfortable and dry for any activity or occasion.

Breathable Plus-Size Panties

Along with cotton, mesh is another material that’s breathable and keeps you cool and dry while you’re wearing it. These high-waisted, moisture-wicking panties are a blend of both materials and are perfect for everyday wear.

Reviewers say they love how comfy these panties are, especially that they don't dig into the skin in the area between the crotch and the thighs. Customers also say that they're pretty stretchy, so they move with you as opposed to riding up. One thing reviewers didn't love, though, is that the panties you receive are not as vibrant or in the same colors as the panties depicted.

If you buy these panties, one other thing to keep in mind is that the sizes are unique to the brand, so pay attention to the size chart linked in the item description. For example, a size nine is equivalent to a 3X.

The inner lining on this underwear is made of a super absorbent fabric that's built to be worn on your period. Some of our editors have personally ditched pads and tampons forever for these period panties, and they've been a total game changer for comfort. They're also great for heavy sweaters to stay dry while working out.

WH's supervising video producer, Erika Santos, is a big fan of these. "I like that I feel more at peace when I where period-proof panties," Santos says. "I have very heavy periods and have never leaked using these." Her only caveat with these is that they sometimes hold on to odor even after being washed. So, you should follow care instructions carefully.

Customers who have tried these panties say that their high-waist and soft, stretchy feel make them super comfortable. Invisible Edge Microfiber Thong

If you're looking for breathable underwear that won't show through your clothes, these thongs from Jockey are it. The panties, which have a second-skin feel, are a favorite of commerce editor Jasmine Gomez.

"These are legit my everyday go-to thongs," she says. "They don't dig in or shift throughout the day, which is a major plus to me as a person who's prone to chafing down there in poorly-fitted underwear." Gomez also loves that they're fairly affordable and come in a bunch of styles and colors. Her only caveat is that they may feel a little too stretchy over time, which isn't great for those looking for a tucked-in feel.

How we chose the best breathable underwear

For the past few months, Women’s Health editors, including our commerce editor Jasmine Gomez, vetted the bestselling, top-rated breathable underwear. Our team consulted an ob-gyn, and then evaluated thousands of customer reviews and ratings to narrow down the best breathable underwear for a wide range of needs and preferences.

How do underwear materials affect vaginal health?

Being mindful of what your underwear is made of can save you from a host of vaginal health issues like infections and irritation. That's because yeast and bacteria tend to thrive in moist, dark places, says Alyssa Dweck, MD, an ob-gyn based in Westchester, New York.

Certain underwear materials can create that damp environment. "Breathability is really important," Dr. Dweck says. “Synthetic materials like Lycra and nylon can be confining and non-breathable and can lead to irritation, rashes, and infection, especially if you’re already prone to those things.”

That's not to say that you'll definitely get an infection if you wear underwear made of these materials. But, there are fabrics that are airier and will make your vagina much, much happier and cut down on your risk.

What material should my underwear be made of?

Cotton is the best choice when it comes to everyday wear, according Dr. Dweck says. It’s the most absorbent material and the best for promoting breathability. (And, yup, they're usually pretty affordable, too.)

If you have sensitive skin or you’ve been dealing with recurrent, persistent infections or irritation, opt for *organic* cotton or another natural material, Dr. Dweck says. Otherwise, conventional cotton is just fine.

The exception to the cotton rule is when you're working out. It's fine, and probably even beneficial, to forgo cotton when you're exercising. The best underwear to wear while working out tends to be any kind made of moisture-wicking materials, Dr. Dweck says.

Are thongs a good choice?

The good news: Thongs don't make a difference in promoting or decreasing the chance of infection or irritation. As with other styles, it’s ultimately the materials that matter. Still, if you opt for a thong, go for one that fits properly and has a cotton crotch panel, as a too-tight thong can lead to chafing, particularly if it’s a g-string style, Dr. Dweck says. (Ouch.)

And, for anyone out there who loves forgoing undies altogether, there's even better news. Occasionally, going commando is even better than opting for cotton, says Dr. Dweck. If you’re prone to vaginal infections, sleeping without underwear may help to curb them.
