United Health Foundation Awards $550,000 to Daughters of Charity Health Centers' St. Cecilia Location
Grant supports DCHC's Patient-Centered Medical Home Model and Neighborhood Health Partnership
St. Cecilia site provides critical health and social services for people in Bywater neighborhood
NEW ORLEANS--(BUSINESS WIRE)-- Daughters of Charity Health Centers (DCHC) announced it has received a $550,000 grant from United Health Foundation. The grant will support DCHC's Patient-Centered Medical Home Model and Neighborhood Health Partnership program at this location.
United Health Foundation awarded the grant during a community event today at the site, which is located in the Bywater neighborhood in New Orleans' Ninth Ward. The foundation has given nearly $7 million in grants to Daughters of Charity Health Centers since 2006.
"Our St. Cecilia location provides critical health care services to the people of the Bywater neighborhood and surrounding areas, and we are able to deliver these services due in large part to support from United Health Foundation," said Michael Griffin, president and CEO of Daughters of Charity Health Centers. "With the foundation's support, we continue to implement an integrated, innovative model of health care that effectively advances our mission to improve the health and well-being of our community."
Daughters of Charity Health Centers' Patient-Centered Medical Home Model, which involves a team of health professionals including physicians, nurse care managers, behavioral health managers and case managers, provides an elevated level of care to residents in the area who face health and social challenges. The model re-establishes the important patient-physician relationship by having patients receive long-term coordinated and comprehensive care from a primary care physician, or "medical home," instead of episodic care from various specialists who are less familiar with their medical history.
The Neighborhood Health Partnership (NHP) is an outreach program that provides health screenings and education, and directs people to Daughters of Charity Health Centers for primary and preventive care.

Since 2003, United Health Foundation has committed nearly $40 million in funding to community health centers across the country.
