Catholic Health Initiatives Implements Conifer Health Solutions' Value-Based Care Services in Two States

New agreement includes population-health management and data-analytics services

FRISCO, Texas--(BUSINESS WIRE)-- Catholic Health Initiatives (CHI), the nation's third-largest faith-based health system, has chosen Conifer Health Solutions' Value-Based Care services to support clinically integrated networks now being developed by CHI in Kentucky and the Tacoma/Puget Sound area of Washington state.

The partnership provides CHI with population-health management and data-analytics tools, including a care-management platform, integrated Web portal, and self-service clinical analytics designed to enhance the medical-management process and add value for patients and providers. Conifer Health will also help each market network conduct predictive modeling, stratify risk, and leverage its data repository as part of CHI's overall population health-management strategy. Conifer Health's Value-Based Care solutions, which can be customized on a market-by-market basis, include financial risk management, clinical integration and population health management.
