Health Net Identification Cards Sent to Members' Former Addresses

LOS ANGELES--(BUSINESS WIRE)-- Health Net of California, Inc., a wholly owned subsidiary of Health Net, Inc. (NYS: HNT) , today announced that on May 3, 2013, Health Net became aware that some California member identification cards inadvertently were mailed to members' former addresses. Health Net has been diligently investigating the issue and is working closely with the California Department of Health Care Services (DHCS) on this matter.


The mailings occurred in April and May 2013, as part of a program transition of Health Net members to the Health Net Medi-Cal program. The information on the identification cards did not contain financial or medical information; however, the identification cards did contain the member's name, identification number and the name of the primary care physician. Health Net believes that the programming issue that resulted in the identification cards being mailed to a member's former address has been addressed.

Health Net is in the process of notifying affected members that the information sent to their former addresses included their plan identification number. In addition, Health Net has notified the appropriate governmental authorities within the required time frames.

Health Net has established a call center to answer affected members' questions. The toll free call center number is 1-855-398-6438 and is available Monday through Saturday between 6 a.m. and 6 p.m. Pacific time.
