Get a free sample of Crest Pro-Health toothpaste through Walmart when you take a quick quiz about gingivitis. Answer two questions about the health of your gums and if you appear to have symptoms of gingivitis, you will get the free sample. If you don't, Crest will offer you a sample of another toothpaste. In my case, I got an offer for Scope Outlast Toothpaste.

The free sample form asked for my name, address, e-mail, and birth year. There were also three multiple choice questions and an option to sign up for Walmart's email list. It's unclear how long this sample will be around.

Caveats: while supplies last. One sample per household. Samples are only available to consumers in the United States. Expect the toothpaste to take a few weeks to get to you once you submit the form.
